If you would like to book your assessment, please fill in and submit the downloadable booking form below and then email it to us at matrix@emqc.co.uk.

Before you begin filling in this form, in addition to information about your organisation, you will also need to have to hand the following information:

The total number of people involved in the management and delivery of your organisation and its information, advice and guidance service or services. This should also take into account any senior managers, board members that are involved in planning your organisation's overall aims and objectives, and the total number of people delivering your service. This should include all part-time, full-time staff and volunteers
The total number of clients that you deliver to on an annual basis. This should include all your client groups for example your service may be available to your staff as well as to members of the general public. If you haven’t yet completed a full year in business this should be an estimate of the number of clients that you are planning to deliver to over your first year
